Some care plan<br>patients are eligible for referrals to allied health providers at a subsidised cost. We also offer health assessments for over 75&#8217;s, asthma plans, healthy heart checks, anaphylaxis action plans and diabetes reviews. Please ask your doctor if you are eligible for a care plan.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Preventative Health Checks<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Over time we all become more vulnerable to illnesses. To reduce risk and catch warning signs early, we recommend having the following regular health checks:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Heart disease risk assessment every 2 years<\/li>\n\n\n\n<li>Blood pressure check every 2 years<\/li>\n\n\n\n<li>Cholesterol and lipids check every year <\/li>\n\n\n\n<li>Blood glucose test every year <\/li>\n\n\n\n<li>Mammogram every 2 years (women) <\/li>\n\n\n\n<li>Skin cancer checks<\/li>\n\n\n\n<li>Cervical screening test every 5 years (women) <\/li>\n\n\n\n<li>Bowel cancer check every 2 years<\/li>\n\n\n\n<li>Osteoporosis risk assessment every year<\/li>\n\n\n\n<li>Flu vaccination every year <\/li>\n\n\n\n<li>Dental check-up every year <\/li>\n\n\n\n<li>Falls assessment (over 65) <\/li>\n\n\n\n<li>Hearing test every year (over 65) <\/li>\n\n\n\n<li>Eye test every 2 years, and every year on reaching age 65<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Health Assessments At BMCOL we offer comprehensive and individualised GP Management Plans which arecompleted by a GP for patients diagnosed with a chronic health condition.
